What Is the EX-00 Form?

The EX-00 (Solicitud de Visado Nacional) is Spain's official national visa application form, required for all long-stay visa types — including the student visa (type D). How to Complete the EX-00 Step by Step

Follow these steps precisely. The form must be completed in Spanish, printed on A4 paper, and signed in ink before your appointment.

Download the Current Version

Always download the EX-00 form directly from your Spanish consulate's official website or the Spanish Ministry of Foreign Affairs. Forms circulating online may be outdated. Use the version published by the consulate responsible for your jurisdiction — the US, UK, Australian, and Canadian consulates all publish the current form on their websites.

Complete All Fields in Spanish

The EX-00 must be completed entirely in Spanish. Your name, nationality, address, and purpose of stay must all be written in Spanish. For your nationality, write the Spanish word (e.g. "Estadounidense", "Británica", "Canadiense", "Australiana"). For your country of birth, use the Spanish place name (e.g. "Londres, Reino Unido" not "London, United Kingdom").

Enter Your Purpose of Stay Correctly

In section 21 (Motivo del viaje / Purpose of stay), write Estudios. Also include the name of your Spanish institution and the start and end dates of your course. This section is critical — if it says anything other than Estudios, your visa type will be mismatched and the application will fail.

Fill in Your Proposed Address in Spain

You must provide your intended address in Spain — the street address, city, and postal code where you plan to live. This should match the accommodation proof you are submitting. If you have booked student accommodation or a rental, use that address. If staying with a host family, use their address (you may also need a host declaration letter).

Sign by Hand Before Your Appointment

Print the completed form on A4 paper and sign the declaration section with a pen. Do not use a digital or printed signature. Most consulates require the form to be signed but not dated until the day of your appointment — check your specific consulate's instructions. Bring two signed copies where possible.

Attach One Passport-Sized Photograph

Attach a recent passport-sized colour photograph to the designated box on the form. The photo must be taken against a white background, face forward, no glasses, taken within the last 6 months. The same photograph requirements as a passport photo apply — biometric standard.
